Ticket to Ride Form - for Fairview/Trailview Students

(Click this link to download the form.)

Fairview and Trailview Schools have implemented a new "Ticket to Ride" that will provide the
transportation department with the information needed to deliver our students to a location other than the
child's typical after school destination.

The tickets will only be necessary if there is a CHANGE in typical after school transportation and must be
filled out completely, including the phone number where the school will be able to reach you for any
questions that may arise.

If after school plans change during the school day, please notify the office before 2:00.  A
"Ticket
to Ride"
,like the one shown in the link above, will be completed by the information you provide.
This form can also be downloaded from our website, completed with your signature and sent to school
with the child.

TRiO UPWARD BOUND

Upward Bound is a federally-funded college preparatory TRiO program through the U.S. Department of
Education and hosted by Anoka-Ramsey Community College. The program is designed to help eligible high
school students succeed in completing high school and prepare them to go to college.

Students currently in grades 9-11 are eligible for the program by meeting federal income guidelines and/or
coming from a family where neither parent has earned a four-year degree.  Students are selected to join
the program based on having a need for the services and potential to attend and succeed in college.

If parents or students are interested in applying or would like more information
about the program, call the Mora Upward Bound Advisor, Kelsey Derner, 763-433-1848.
